www.advancingqualitynw.nhs.uk is the public web site of the Advancing Quality programme. Advancing Quality involves all NHS hospitals in the North West but is operated by NHS North West. As an NHS organisation, we are committed to maintaining the security of any information you supply to us.

Uses of personal data:
Advancing Quality works with NHS organisations to improve the quality of patient care. To do this we use various sources of information. Sometimes we use personal data related to patient treatments, outcomes, and experience so that we can better measure and improve the quality of health services. We only do this with the consent of the patient or where we have been granted Section 251approval by the National Information Government Board for Health and Social Care via their Ethics and Confidentiality Committee.
Types of Personal Data Used by AQ:
Patient Experience Measures – Patients receiving care on an Advancing Quality clinical pathway (Heart Attack, Heart Failure, Heart Bypass, Pneumonia and Hip and Knee Surgery) are asked eight questions to measure the quality of care they have received in hospital.
Patient Reported Outcome Measures – Hip and Knee Surgery patients are asked to participate in surveys that are designed to help us understand whether the surgery was a success from the patients’ perspective.
Heart attack treatments – For heart attack patients we receive details of how each patient was treated. This information helps us to check that all patients are being treated in the way that clinical staff across the North West have agreed is appropriate. This information is obtained from the Myocardial Infarction National Audit Project.
Coronary Artery Bypass Graft (CABG) risk factors – for CABG patients we receive something called the Euroscore. Euroscore is a method of predicting the chances of dying during or shortly after undergoing heart surgery. If you are a CABG patient this will have been discussed with you during your hospital stay. AQ uses the Euroscore so that we can calculate mortality rates in this clinical area.
Disclosures of your information:
We will never pass your personal information to anyone else, except where we are required or permitted to do so by law. We will use and disclose information in aggregate (so that no individual patients are identified) for the purpose of improving the quality of patient care.
